INSTRUCTIONS

Step 1

Download the Membership Registration/Update Form through the following link:

CY 2024 AGMM Membership Form

Please take note of the attachments for submission together with the Membership Registration/Update Form.

Step 2

Fill up the Membership Form and remember to save the files in PDF format using the following file name: PEMC_2024_NAME OF MEMBER

Submit the Form together with Attachments in a zip file.

Step 3

Download the Nomination Form through the following link:

PEM Board - Nomination Form

FOR INDEPENDENT BOARD NOMINEE

Please take note of the attachments for submission together with the Nomination Form.

Step 4

Fill up the Nomination Form and remember to save the files in PDF format using the following file name: PEMC_2024_BOD_Nominees_NAME OF NOMINATING MEMBER.

Submit the Form together with Attachments in a zip file.

Step 5

Email your accomplished forms and their attachments to pemc_ocs@wesm.ph

MEMBERSHIP REGISTRATION/ UPDATE FORM

• Valid Government I.D. of the Authorized Representative
• Board Resolution/Secretary’s Certificate of the Authorized Representative
• If applicable:
• Certificate of Compliance
• Latest Report filed with the ERC on number of Customers
• Data Privacy Form

NOMINATION FORM

• 2x2 Photo of the Nominee
• Valid Government I.D. of the Nominee
• Picture of the Nominee Holding the Valid Government I.D.
• Notarized Nomination Form
• Notarized Nominee Undertaking
• Updated Curriculum Vitae
• Data Privacy Form
